Grant Extension Request

Please use this form to request an extension of the end of your current grant period as set in your grant agreement.
If approved, all funds from this grant must be spent or encumbered (obligated in a contract, purchase order, or other formal commitment) by the end of the new grant period listed below.

If you will not be able to spend or encumber the entire amount awarded or complete a majority of the funded activities within the grant period, the grantee should request a grant period extension to the date when all funds will be spent/encumbered and/or activities completed.

Extensions must be requested before the grant period ends. Failure to provide an extension request prior to the end of the grant period could result in forfeiture of all remaining funds. If your request for extension is not approved, you will be required to return all remaining grant funds within 30 days from date of notice from the Foundation.
